Liquid Silicone With Ease and Precision

BACKGROUND: Since the Food and Drug Administration approved the use of the 1000
centistoke liquid silicone, Silikon 1000, for intraocular injection, the off-label
use of this injectable silicone oil as a permanent soft-tissue filler for facial
rejuvenation has increased in the United States. Injecting liquid silicone by
the microdroplet technique is the most important preventive measure that one
can use to avoid the adverse sequelae of silicone migration and granuloma formation,
especially when injecting silicone to improve small facial defects resulting
from acne scars, surgical procedures, or photoaging.
OBJECTIVE: To introduce an easy method for injecting a viscous silicone oil
by the microdroplet technique, using an inexpensive syringe and needle that
currently is available from distributors of medical supplies in the United States.
METHOD: We suggest the use of a Becton Dickinson 3/10 cc insulin U-100 syringe
to inject Silikon 1000. This syringe contains up to 0.3ml of fluid, and its
barrel is clearly marked with an easy-to-read scale of large cross-hatches.
Each cross-hatch marking represents either a unit value of 0.01 mL or a half-unit
value of 0.005ml of fluid, which is the approximate volume preferred when injecting
liquid silicone into facial defects. Because not enough negative pressure can
be generated in this needle and syringe to draw up the viscous silicone oil,
we describe a convenient and easy method for filling this 3/10cc diabetic syringe
with Silikon 1000.
RESULTS: We have found that by using the Becton Dickinson 3/10cc insulin U-100
syringe, our technique of injecting minute amounts of Silikon 1000 is facilitated
because each widely spaced cross-hatch on the side of the syringe barrel is
easy to read and measures exact amounts of the silicone oil. These lines of
the scale on the syringe barrel are so large and clearly marked that it is virtually
impossible to overinject the most minute amount of silicone.
CONCLUSION: Sequential microdroplets of 0.01cc or less of Silikon 1000 can be
measured and injected with the greatest ease and precision so that inadvertent
overdosing and complications can be avoided.

πάρα πολύ η χρήση του μόνιμου αυτού εμφυτεύματος και για άλλες χρήσεις όπως
δυσμορφίες προσώπου από ουλές ακμής, χειρουργικές επεμβάσεις, φωτογήρανση κ.λπ.
Επίσης χρησιμοποιείται για την βελτίωση της αισθητικής εμφάνισης (αύξηση όγκου
χειλέων, γέμισμα ρυτίδων κ.λπ).
Σε αυτό το άρθρο γίνετε πρόταση χρήσης της Silikon 1000 με την τεχνική των μικροσταγονιδίων
με σύριγγα ινσουλίνης Becton Dickinson 3/10cc η οποία διαφέρει από την κλασική
σύριγγα ινσουλίνης στο ότι αρίθμηση είναι ανά 0,05ml (η κάθε μικροσταγόνα θεωρείτε
περίπου 0,01cc), έτσι είναι σχεδόν αδύνατο να ενεθεί μεγαλύτερη ποσότητα ελαχιστοποιώντας
έτσι πιθανές παρενέργειες, επιτυγχάνοντας το επιθυμητό αποτέλεσμα.

Από το 1964 έχουν περιγραφεί διάφορες χειρουργικές τεχνικές για τη θεραπεία
της επίμονης, αλλά σταθερής λεύκης. Μεταξύ των τεχνικών αυτών έχουν αναφερθεί:
- punch grafting (μοσχεύματα punch)
- epidermal grafting (επιδερμιδικά μοσχεύματα με την τεχνική της φυσαλίδας)
- split skin grafting (μερικού πάχους μοσχεύματα)
- grafting of non-cultured epidermal suspension (μοσχεύματα μη καλλιεργημένων
επιδερμιδικών κυττάρων)
- transplantation of in vitro cultured epidermal suspension of melanocytes (μεταμόσχευση
εργαστηριακά καλλιεργημένων επιδερμιδικών κυττάρων)
Transplantation of human melanocytes (μεταμόσχευση ανθρώπινων μελανοκυττάρων).
Ανεξαρτήτως τεχνικής παρατηρήθηκαν καλά αποτελέσματα στην τμηματική αλλά σταθερή
λεύκη, ενώ στη λεύκη με ραγδαία επέκταση τα αποτελέσματα ήταν απρόβλεπτα. Η
ύπαρξη φαινομένου Koebner αποτελούσε κακό προγνωστικό παράγοντα όσον αφορούσε
την επιτυχία της τεχνικής.

BACKGROUND: Botulinum toxin
type A (BTX-A, BOTOX) is an excellent therapeutic option for hyperkinetic facial
lines. It improves wrinkles by relaxing the muscles of facial expression, which
underlie the rhytids. Periocular wrinkles such as lateral orbital rhytids respond
well to treatment. BOTOX can be used in the lower eyelid to improve wrinkles
and widen the eye.
OBJECTIVE: To determine whether there is additional benefit in using more than
2U of BTX-A to improve infraorbital wrinkles and widen the eye.
METHODS: Nineteen women had BTX-A injected into the orbicularis oculi muscle.
Eleven women had 4U injected into the lower eyelid bilaterally, 3mm below the
ciliary margin, and 12U of BTX-A injected into one lateral orbital (crow's feet)
area. Eight patients had 8U injected bilaterally into the lower eyelid and 12
U placed unilaterally into the crow's feet. Physicians and patients independently
evaluated the degree of improvement (grade 3=dramatic improvement, grade 2=moderate
improvement, grade 1=mild improvement, and grade 0=no improvement). Single investigator
analysis was used to measure, in actual millimeters, the amount of increase
in palpebral aperture. Side effects were noted.
RESULTS: Improvement was noted in lower eyelid wrinkles by both physicians and
patients at both dose groups. When only the lower lid was injected, patients
reported an improvement of 1.18 with 4U and a grade of 1.63 with 8U. When both
the lower eyelid the lateral orbital area were treated, an improvement of 1.73
was reported with 4U and a grade of 2.25 reported with 8U in the lower eyelid.
Physician evaluations had grades of 1.85 for 4U alone and 1.85 with 8U alone.
Grades of 2.35 and 2.25 were obtained for 4U plus 12U and 8U plus 12U, respectively.
An increase in palpebral aperture (IPA) occurred in all subjects. Subjects who
received 4U in the lower eyelid alone had a 1.8mm IPA at rest and a 2.6mm increase
at full smile. Subjects who received 8U of BTX-A alone in the lower lid had
an IPA of 2.2mm at rest and 2.9mm at full smile. Eyes treated with 12U in the
bilateral orbital area plus 4U in the lower eyelid had an IPA of 2.2 at rest
and 4.5mm at full smile. Those treated with 8U in the lower lid plus the crow's
feet had an IPA of 1.5 at rest and 4.0 at full smile. Side effects increased
with dosage, with eight of eight subjects in the 8U dose groups reporting bothersome
side effects such as lower eyelid edema and incomplete sphincter function.
CONCLUSION: A dose-response curve is seen with increasing doses of BTX-A used
in the lower eyelid. Treatment of the lateral orbital area in combination with
the lower lid produces a synergistic response at lower doses, but at higher
doses, a plateau effect is suggested. Although increasing doses of BTX-A increases
eye widening, unattractive results and side effects are seen at higher doses.
The authors recommend that lower 2 or 4U doses of BTX-A be used in the lower
eyelid and specifically discuss techniques.

έκφρασης χαλαρώνοντας τους μύες του προσώπου. Περιοφθαλμικές ρυτίδες όπως είναι
οι πλάγιες (πόδι χήνας) ανταποκρίνονται επίσης πολύ καλά.
Oι συγγραφείς εδώ προτείνουν και τη χρησιμοποίησή του στο κάτω βλέφαρο για βελτίωση
των ρυτίδων και αύξηση του ανοίγματος του ματιού. Αναφέρουν ότι η αγωγή των

Imiquimod 5% Cream for
Keloid Management

BACKGROUND: Keloid treatment
represents a therapeutic challenge. New adjuvant therapy is needed to reduce
the high recurrence rate (50%) of excised keloids.
OBJECTIVE: To describe the method for using imiquimod 5% cream in the prevention
of keloid recurrence after surgery.
METHODS: This is a review of the scientific rationale and clinical experience
of using imiquimod 5% cream for keloid management.
CONCLUSION: Topical application of imiquimod 5% cream after surgery reduces
keloid recurrences.
Σε αυτό το άρθρο αναφέρετε μια θεραπευτική πρόκληση. Χρησιμοποιώντας κρέμα ιμικουιμόδης
5 % (τοπικό ανοσοτροποποιητικό φάρμακο) αμέσως μετά τη χειρουργική αφαίρεση

Σε 12 περιστατικά που χρησιμοποιήθηκε και μετά από 6 μήνες, δεν παρατηρήθηκε
καμία υποτροπή σε 10 ασθενείς από τους 11 που ολοκλήρωσαν τη μελέτη.
Βέβαια, οι συγγραφείς αναφέρουν ότι χρειάζονται ακόμα πολλές μελέτες.

BACKGROUND: Training models
are necessary for development of many skills in medical education.
OBJECTIVE: To develop a training model for improvement of simple surgical procedure
skills.
METHODS: A convex, rectangular, smooth-surfaced wooden plate covered by first
cotton material and then polyethylene stretch film was used. Skins of rats that
had been used previously for experimental studies were attached on the wooden
plate. In order to see efficacy of our model, we decided to use it in a cutaneous
surgery workshop. Theoretical and practical information about surgical sutures,
punch biopsy, excisional biopsy, rotational flap, and Z-plasty procedures was
given to the participant physicians. Surgical skills of participants were scored
before and after the practical part of workshop.
Results: Thirty-three physicians were enrolled voluntarily to the workshop.
All participants showed statistically significant increase in scores (p<0.05).
CONCLUSION: The similarity to human skin and the easy setup make this training
model an ideal teaching tool to improve the skills of physicians for simple
cutaneous surgery.
Στο άρθρο αναφέρεται ο σχεδιασμός ενός εκπαιδευτικού μοντέλου για εκπαίδευση
των ιατρών στην χειρουργική δέρματος. Χρησιμοποιήθηκε δέρμα ποντικού (από ποντίκια
που είχαν χρησιμοποιηθεί για άλλες εργαστηριακές μελέτες) το οποίο είχε επικολληθεί
σε μια ξύλινη πλακέτα.
Το μοντέλο αυτό χρησιμοποιήθηκε σε ένα σεμινάριο δερματοχειρουργικής για δερματικές
βιοψίες (punch και ανοικτές), κρημνούς, μοσχεύματα, τεχνικές συρραφής και ζητήθηκε
από τους συμμετέχοντες ιατρούς να το αξιολογήσουν.
Τα συμπεράσματα ήταν ότι το μοντέλο προσομοίαζε πάρα πολύ με το ανθρώπινο δέρμα
και αποτέλεσε ένα πολύ καλό διδακτικό μέσο για εκπαίδευση στη χειρουργική δέρματος.

of subungual hematoma

BACKGROUND: The most commonly
used treatment method for subungual hematoma is nail trephining, which has some
disadvantages.
OBJECTIVE: To introduce a very simple and well-tolerated treatment technique
for this common problem.
METHODS: We used an extra-fine, 29-gauge insulin syringe for evacuation. We
inserted the needle very close to the nail plate to minimize pain. We drained
the blood from the hyponychium under the nail plate using this extra-fine needle.
RESULTS: The technique described herein is very fast and simple, and the patients
tolerate it well. It is particularly successful for the treatment of smaller
subungual hematomas of the second, third, and fourth toenails, in which the
trephining is harder.
CONCLUSION: For these reasons, we suggest this technique as a practical alternative
to the traditional nail trephining methods.
Το άρθρο αυτό περιγράφει μια πολύ απλή τεχνική για την αναρρόφηση ενός υπονύχιου
αιματώματος χρησιμοποιώντας σύριγγα ινσουλίνης με βελόνα 29G. Η βελόνα εισέρχεται
στο υπονύχιο παράλληλα με την ονυχοφόρο πλάκα και γίνεται αναρρόφηση του αιματώματος
σχεδόν ανώδυνα. Χαρακτηριστικά αναφέρεται ότι ο πόνος είναι λιγότερος από ένα
τρύπημα για στελεχιαία αναισθησία του δακτύλου. Συνιστάται κυρίως για μικρά
αιματώματα 2ου 3ου και 4ου δακτύλου άκρου ποδός.

Treatment of Nonmelanoma Skin Cancer in the Medicare Population

BACKGROUND: Nonmelanoma
skin cancer (NMSC) is the most common malignancy of white populations. Different
surgical treatment options can be used to treat these tumors, depending on the
tumor characteristics and setting.
OBJECTIVE: To determine how frequently different specialists use the different
types of surgical options available for the treatment of NMSC using a 1998-1999
sample of Medicare claims data.
METHODS: Episodes of care of NMSC were identified by pathology claims with a
diagnosis of NMSC. The surgical interventions performed within 7 days before
the pathologic diagnosis of NMSC were then analyzed. The numbers of patients
and the variety of surgical treatments were categorized by specialty to include
dermatology, general surgery, and plastic surgery.
RESULTS: Dermatologists managed 82% of the NMSC episodes. Dermatologists used
a wider range of different treatment options than other specialists and performed
90% of the biopsies, 56% of the excisions, 95% of the destructions, and 100%
of the Mohs micrographic surgeries for NMSC.
CONCLUSIONS: Dermatologists identify and manage most of the NMSC in the United
States and offer patients a broad range of surgical options. The use by dermatologists
of office-based surgical methods not commonly used by other specialists may
partially explain previous findings of better NMSC outcome by dermatologists.
The low level of use of specific surgical options by some specialists may indicate
the need for greater training/exposure to these methods.
Στο άρθρο αυτό γίνεται μια ανασκόπηση (1998-1999) γύρω από την αντιμετώπιση
μη μελανωματικών δερματικών καρκίνων από διαφορετικές ιατρικές ειδικότητες (Δερματολόγοι,
Γενικοί χειρουργοί, Πλαστικοί χειρουργοί) στην Αμερική.
Συμπερασματικά, οι Δερματολόγοι χειρίστηκαν το 82 % των μη μελανωματικών δερματικών
καρκίνων χρησιμοποιώντας μεγαλύτερο φάσμα θεραπευτικών προσεγγίσεων από τις
υπόλοιπες ειδικότητες, πραγματοποιώντας το 90 % των βιοψιών, το 56 % των αφαιρέσεων,
το 95 % των καταστροφών των υπόπτων βλαβών με διάφορες μεθόδους (Ηλεκτοχειρουργική,
Κρυοχειρουργική, Απόξεση, Laser κ.λπ.) και το 100 % Mohs μικρογραφικής χειρουργικής.
Επίσης γίνεται αναφορά στο ότι οι Δερματολόγοι όχι μόνο διέγνωσαν και χειρίστηκαν
τους περισσότερους ασθενείς με μη μελανωματικούς δερματικούς καρκίνους, αλλά
και προσέφεραν περισσότερες χειρουργικές τεχνικές ιατρείου στους ασθενείς.
